Description
Running centrally within the deep posterior compartment, the posterior tibial artery descends from the popliteal region beneath the tendinous arch of the soleus, then courses inferiorly along the posterior surface of the tibia toward the medial malleolus. Superficial to it lie the gastrocnemius and soleus, while deeper neighbors include tibialis posterior and flexor digitorum longus, with flexor hallucis longus occupying a more lateral position toward the fibula. The tibia and fibula frame the vessel medially and laterally, and the accompanying posterior tibial veins track closely alongside the artery as it approaches the tarsal tunnel. Posterior orientation matters here because the posterior tibial artery becomes a key landmark as it passes posterior to the medial malleolus, where it is palpated for the posterior tibial pulse and where it bifurcates into the medial and lateral plantar arteries. Clinically, this segment is evaluated in peripheral arterial disease and diabetes, and it sits in the operative corridor for tarsal tunnel release and medial ankle approaches, where the tibial nerve and flexor tendons share tight confines. Distal perfusion depends on it.
