Description
Presented is a lumbar vertebra rendered in posterior orientation, with the spinous process projecting posteriorly and inferiorly from the vertebral arch and the paired transverse processes extending laterally from the pedicle junctions. Superior articular processes rise at the posterolateral corners of the arch, their facets oriented to meet the inferior articular facets of the vertebra above, while the vertebral foramen sits centrally behind the vertebral body. Thick laminae converge toward the midline to form the base of the spinous process. Bony surface texture is retained, making ridges and pits that correspond to muscle and ligament attachment sites easy to read. Posterior lumbar anatomy matters because this is the corridor used in most posterior spinal exposures, and the landmarks you palpate first are the spinous process, lamina, and facet (zygapophyseal) joints. Facet joint arthropathy and hypertrophy at L4 to L5 and L5 to S1 are common pain generators and can contribute to foraminal or lateral recess stenosis when combined with disc degeneration. Orientation of the articular facets also frames the biomechanics of degenerative spondylolisthesis, typically at L4 to L5, where sagittal facet alignment permits anterior translation.
