Description
Running longitudinally along the posterior neck, the spinalis cervicis is presented in lateral profile as the most medial column of the erector spinae, closely apposed to the cervical spinous processes and deep to the more laterally positioned longissimus cervicis. Superiorly, its fibers ascend toward the upper cervical vertebrae, while inferiorly they blend with the thoracic portion of spinalis near the cervicothoracic junction. Anterior landmarks remain readable through the semi-transparent integument, including the mandible and maxilla with dentition, the hyoid bone at the anterior neck, and the clavicle with the first ribs; the sternocleidomastoid lies superficial and anterolateral to the cervical spine. The external ear anchors orientation at the lateral skull. Clinically, this view clarifies why focal tenderness just lateral to the cervical spinous processes often reflects posterior paraspinal strain rather than sternocleidomastoid pathology, and it helps separate midline extensor mechanics from the more lateral longissimus and iliocostalis components. Manual therapy, trigger point mapping, and posterior cervical approaches all depend on keeping this medial muscle column straight in your mind when palpating from the mastoid region down toward C7 and T1. Small muscle, big implications.
