Description
Posterior thoracic anatomy is presented with both scapulae highlighted against the axial skeleton of an adult female. Each scapula lies posterolateral to the rib cage, spanning roughly the second through seventh ribs, with the medial (vertebral) border paralleling the thoracic spinous processes and the inferior angle pointing inferomedially toward the lower thorax. Superiorly, the scapular spine would course laterally toward the acromion, while the lateral border thickens toward the glenoid region where the proximal humerus would articulate at the shoulder. The occipital bone, thoracic vertebrae, ribs, and proximal upper limbs provide clear positional landmarks. Scapular position on the posterior chest wall matters because many shoulder complaints originate from scapulothoracic dyskinesis rather than the glenohumeral joint itself, and this rear view makes asymmetry easy to teach and to recognize. Medial border prominence and inferior angle lift are classic for scapular winging, most often from serratus anterior weakness after long thoracic nerve injury, but also from spinal accessory nerve lesions affecting trapezius. Fractures of the scapular body are uncommon and usually indicate high energy trauma, so correlating the scapula to the rib cage and thoracic spine helps frame mechanism and associated injuries. Use this illustration in gross anatomy and kinesiology modules when introducing the scapulothoracic articulation, scapular landmarks, and the resting position of the shoulder girdle in anatomical position.
