Description
Posterior osteology of the female shoulder girdle is presented with both scapulae highlighted against the axial skeleton. Each scapula lies on the posterolateral thoracic wall, its medial (vertebral) border paralleling the spinous processes while the lateral angle thickens toward the glenoid region adjacent to the proximal humerus. The scapular spine runs obliquely from medial to lateral, expanding superiorly into the acromion, which arches over the glenohumeral joint and meets the clavicle at the acromioclavicular articulation. Vertebrae, ribs, pelvis, and the long bones of the upper limb remain visible for orientation. Clear landmarks. For teaching and clinical communication, the posterior scapula is where bony contours map directly to muscle compartments and common pain generators. The scapular spine separates the supraspinous and infraspinous fossae, guiding rotator cuff localization in examinations and in posterior approaches to the shoulder, while the acromion’s lateral edge is a reliable surface reference in subacromial impingement and acromioclavicular joint injury. Scapular winging is easier to conceptualize from this angle because abnormal prominence of the medial border relates to serratus anterior dysfunction (long thoracic nerve) or trapezius weakness (spinal accessory nerve), both assessed by comparing left and right scapular position during active elevation. Orthopedic and physical therapy curricula use this view to teach scapulothoracic mechanics, scapular dyskinesis patterns, and palpation of the acromion and scapular spine as landmarks for injections and taping. It also fits well in musculoskeletal radiology and anatomy lab manuals when correlating posterior surface anatomy with AP and scapular Y radiographs for fracture orientation and shoulder dislocation workup.
