Description
Posterior skeletal orientation frames both humeri in the context of the shoulder girdle and thorax, with each upper arm bone highlighted from the proximal head and anatomical neck to the distal condylar region at the elbow. Proximally, the humeral head articulates with the glenoid cavity of the scapula, while the greater tubercle sits lateral to the intertubercular sulcus and the lesser tubercle lies more anterior, landmarks that remain readable even from a rear view through their silhouettes and relative prominences. Along the shaft, the lateral and medial borders converge toward the distal humerus, where the lateral epicondyle and the more prominent medial epicondyle bracket the olecranon fossa on the posterior aspect, immediately proximal to the trochlea and capitulum. Scapulae, vertebral column, ribs, pelvis, and lower limbs provide midline and bilateral reference points for comparing left versus right brachium. Posterior orientation is the cleanest way to teach the olecranon fossa and the shaft’s posterior surface, including the region where the radial nerve and profunda brachii artery course in the radial (spiral) groove before piercing the lateral intermuscular septum. That relationship is the clinical trap in midshaft humeral fractures, where radial nerve palsy can present with wrist drop while the fracture line may be subtle on initial radiographs. Orthopaedic planning often relies on these posterior landmarks during a posterior triceps-splitting approach or when positioning plates to avoid iatrogenic nerve injury. A simple view, but unforgiving. Use this asset in gross anatomy and musculoskeletal modules to anchor osteology of the arm, or in orthopaedic and emergency medicine teaching to pair fracture patterns with surface anatomy and neurovascular risk. It also fits well in textbooks and e-learning on upper limb trauma, arthroplasty basics, and comparative left-right alignment in whole-skeleton plates.
