Description
Arising from the right pulmonary artery, the superior segmental artery (A2) courses into the posterior segment of the right upper lobe, running alongside the segmental bronchus and its accompanying veins within the bronchovascular sheath. From the pulmonary trunk at the heart’s base, the pulmonary arteries track laterally toward the hila, then branch intraparenchymally into lobar and segmental vessels that arborize into progressively smaller arterioles. Superiorly, the trachea bifurcates at the carina into the main bronchi, and the right upper lobe bronchus divides into segmental branches that mirror the arterial pattern. Segmental veins lie in the intersegmental planes and return to the superior pulmonary vein, providing a useful contrast to the more centrally placed segmental artery. A2 is a practical landmark in thoracic surgery because segmentectomy for a posterior right upper lobe lesion hinges on correct identification and control of the segmental artery, bronchus, and intersegmental venous drainage. Misidentifying A2 as the apical (A1) or anterior (A3) segmental artery can leave devascularized parenchyma or incomplete resection, and the risk rises when fissures are incomplete or the arterial branching pattern varies.
