Description
Emerging from the hilum of the left lung, the left superior pulmonary vein courses medially and slightly inferiorly toward the left atrium, positioned anterior to the left pulmonary artery and anterior to the left main bronchus in the classic hilar arrangement. Tributaries from the left upper lobe, including venous drainage from the lingular segments, converge to form a short venous trunk before it enters the atrial wall. Nearby, the pulmonary trunk and its bifurcation sit superior and anterior to the left atrium, with the ascending aorta and aortic arch curving superiorly, while the trachea with its C shaped cartilaginous rings lies superior to the great vessels. Color coding separates oxygenated pulmonary venous return from deoxygenated pulmonary arterial flow. Orientation is clear. This relationship matters when you need to teach or plan around the left hilum, where pulmonary veins are commonly encountered first during anterior hilar dissection and video assisted thoracoscopic surgery. The left superior pulmonary vein is a key structure in left upper lobectomy and lingular segmentectomy, and it is also one of the target ostia for catheter based pulmonary vein isolation in atrial fibrillation. On imaging, distinguishing the vein from the left pulmonary artery and bronchus prevents wrong structure labeling and reduces operative risk during stapling or ligation. A tight corridor.
