Description
Running bilaterally along the anterolateral surfaces of the thoracolumbar vertebral bodies, the sympathetic trunks form paired longitudinal chains of ganglia that parallel the vertebral column on the posterior abdominal wall. Segmental white and gray rami communicantes link the trunk to the thoracic and lumbar spinal nerves, while splanchnic nerves course anteroinferiorly from the ganglia toward the preaortic plexuses. Lateral to the spine, the kidneys sit retroperitoneally, with the abdominal aorta slightly left of midline and the inferior vena cava to the right; lymphatic nodes and channels may appear adjacent to the great vessels. Orientation is posterior, with the paravertebral sympathetic chain medial to the renal hila and closely related to the diaphragmatic crura and psoas major margins. This posterior exposure matters because the sympathetic trunk is a key landmark when teaching the organization of the autonomic nervous system and when planning retroperitoneal or lateral transpsoas approaches to the lumbar spine. Misidentifying the chain, or the lumbar splanchnic nerves departing it, can contribute to postoperative sympathectomy-type findings, including altered lower-limb sweating and temperature regulation, and it helps explain referred pain patterns in renal colic that track via sympathetic afferents to T10–L1. A small structure with outsized consequences.
