Description
Running along the posterior aspect of the arm, the triceps brachii tendon is rendered as a thick, cord-like structure converging from the long, lateral, and medial heads toward its distal attachment on the olecranon of the ulna. Proximally, the muscle bellies occupy the posterior compartment of the brachium, with the tendon becoming more distinct as it courses inferiorly toward the elbow. Adjacent bony landmarks, including the posterior distal humerus and proximal ulna, provide orientation for the tendon’s midline position relative to the medial and lateral epicondyles. Skin is kept in place conceptually, so the surface anatomy relationship remains clear. Posterior exposure of the triceps tendon matters in both diagnosis and operative planning because this is the structure tested during resisted elbow extension and palpated in suspected distal triceps rupture. Avulsion at or near the olecranon insertion, sometimes associated with olecranon bursitis or enthesopathy, alters the normal contour and creates a palpable gap proximal to the olecranon. For surgeons, the tendon’s relationship to the posterior elbow and the subcutaneous olecranon guides incision placement for triceps repair and for posterior approaches to the distal humerus, where protecting the ulnar nerve medially becomes part of the same anatomic conversation.
