Emerging as the lateral cheek prominence, the zygomatic (malar) bone is rendered transparent within a lateral skull projection, allowing its articulation lines to be read against the surrounding craniofacial skeleton. Superiorly, its frontal process meets the frontal bone at the frontozygomatic suture near the lateral orbital rim, while posteriorly the temporal process extends to form the zygomatic arch with the zygomatic process of the temporal bone. Medially, the zygomatic bone contributes to the lateral wall and floor of the orbit through its contacts with the maxilla and greater wing of the sphenoid, and inferiorly it relates to the maxillary alveolar region and dentition seen in profile. The mandibular body and ramus sit inferior and posterior, with the upper cervical vertebrae descending from the cranial base. Lateral viewing matters because the zygoma is both a facial buttress and an orbital landmark, and fractures rarely respect a single suture line. Zygomaticomaxillary complex injuries, common after blunt trauma, often involve the zygomaticomaxillary buttress, infraorbital rim, and zygomatic arch, leading to malar flattening, trismus from coronoid impingement, and infraorbital nerve hypoesthesia. Surgeons planning open reduction and internal fixation follow the contour of the lateral orbital rim and frontozygomatic region for plate placement and symmetry checks. Alignment is the whole story.
