Description
Running inferiorly from the jugular foramen, the vagus nerve (cranial nerve X) descends within the carotid sheath, positioned between the internal jugular vein laterally and the internal carotid artery proximally (then common carotid artery) medially. A lateral view places the nerve just posterior to the pharyngeal wall and laryngeal framework, where pharyngeal branches spread across the middle constrictor while the superior laryngeal nerve angles anteroinferiorly toward the thyrohyoid membrane. More caudally in the neck, the recurrent laryngeal nerve is understood to reascend in the tracheoesophageal groove toward the cricoarytenoid region, functionally tying the cervical course of CN X to intrinsic laryngeal musculature. Clinically, this relationship to the larynx is where the anatomy stops being academic. Injury to the recurrent laryngeal nerve during thyroidectomy or anterior cervical approaches can leave unilateral vocal fold paresis, while involvement of the superior laryngeal nerve, external branch, compromises cricothyroid function and reduces pitch control even when the vocal fold still moves. The carotid sheath course also explains why carotid endarterectomy and neck dissection require constant awareness of CN X and its branch points.
