Description
Rising from the subclavian artery at the root of the neck, the vertebral artery ascends superoposterior to the common carotid artery and medial to the internal jugular vein before entering the transverse foramen of C6. From there it courses vertically through the transverse foramina of the cervical vertebrae, its segments framed by the uncinate processes and anterior tubercles that form the bony tunnel of the foramina transversaria. An anterior presentation also places the paired common carotid arteries and their bifurcation into internal and external carotid arteries in the foreground, with the jugular venous channels running lateral in the carotid sheath. Clinical relevance centers on the vertebral artery’s relationship to the cervical spine and to interventions performed from an anterior neck approach. Rotational or extension injuries can provoke vertebral artery dissection, and an anterior view helps teach how symptoms of posterior circulation stroke can originate from a lesion that begins low in the neck, before the vessel ever reaches the foramen magnum. Surgeons and proceduralists also track this vessel when placing anterior cervical instrumentation or performing image guided cervical injections, where a variant entry level (C5 or C7 rather than C6) or tortuosity can increase vascular risk.
