Description

Framed in the right upper quadrant, the gallbladder (vesica biliaris) sits inferior to the visceral surface of the liver, with its fundus projecting anteroinferiorly and its neck tapering posteromedially into the cystic duct. As the sequence progresses, the cystic duct courses medially to meet the common hepatic duct, forming the common bile duct within the extrahepatic biliary tree. Proximal right and left hepatic ducts are typically visible superiorly at the porta hepatis, while the common bile duct descends toward the first part of the duodenum, anterior to the portal vein and in close relationship to the hepatic artery proper within the hepatoduodenal ligament. Clinical relevance centers on the cystic duct junction and the caliber changes along the extrahepatic ducts, where gallstones, postoperative strictures, and iatrogenic injury commonly declare themselves. Animated motion clarifies spatial relationships that are hard to hold in mind during laparoscopy, especially the transition from gallbladder infundibulum (Hartmann pouch region) into the cystic duct and its variable insertion into the common hepatic duct, a setup for misidentification during cholecystectomy and for Mirizzi syndrome when an impacted stone compresses the common hepatic duct. Following the ductal pathway inferiorly also reinforces why cholangitis and obstructive jaundice localize to the common bile duct, and how ERCP targets the downstream outflow at the ampulla of Vater and sphincter of Oddi.
