Description

Centered in the middle mediastinum, the adult male heart appears obliquely oriented, its base directed posterosuperiorly toward the great vessels and its apex pointing anteroinferiorly and leftward toward the fifth intercostal space. The right atrium and right ventricle occupy a more anterior position behind the sternum, while the left atrium sits posteriorly and the left ventricle forms most of the left border and diaphragmatic surface. Across the sequence, the myocardium cycles through contraction and relaxation, and the ascending aorta is shown emerging superiorly from the left ventricular outflow tract before arching posteriorly within the thorax. Placement matters. Relating the chambers and aorta to the anterior thoracic wall clarifies why a blow to the sternum can precipitate myocardial contusion, why pericardiocentesis targets the left xiphocostal angle to avoid the internal thoracic vessels, and why pericardial effusion preferentially compresses the thin-walled right atrium and right ventricle first in tamponade physiology.
