Description
Centered on the female breast, the mammary glandular tissue occupies the superficial fascia anterior to pectoralis major, with Cooper’s suspensory ligaments extending toward the dermis to partition fat and gland. Deep to the breast, striated fibers of pectoralis major form the muscular bed, separated from the gland by the retromammary space. Arterial supply is represented coursing medially and laterally, consistent with perforating branches of the internal thoracic artery and contributions from the lateral thoracic and thoracoacromial systems, while accompanying superficial and deep veins drain toward the axillary and internal thoracic pathways. Cutaneous innervation tracks in from the anterolateral and anteromedial intercostal nerves, with the nipple-areolar complex positioned as a prominent surface landmark. Close inspection of vascular, lymphatic, and neural relationships matters in breast surgery because skin flaps, the nipple-areolar complex, and gland excision planes all depend on preserving predictable perforators and sensory branches. Lymphatic channels in the superficial and deep plexuses preferentially drain toward the axillary nodes (pectoral group) and, medially, to parasternal nodes, the map that underpins sentinel lymph node biopsy and explains contralateral or internal mammary spread in breast carcinoma. Pain patterns and altered nipple sensation after augmentation, reduction mammaplasty, or mastectomy also trace back to intercostal nerve injury. A surgical anatomy staple.
