Description
Anterior thoracic anatomy is rendered with the female mammary glands positioned superficial to the pectoral fascia, extending vertically from the level of the 2nd to 6th ribs and transversely from the lateral border of the sternum toward the anterior axillary line. Glandular tissue sits within the subcutaneous fat, with the nipple and areola centered over the breast mound and the axillary tail (of Spence) projecting superolaterally toward the axilla. Deep to the breast, the pectoralis major forms the dominant muscular bed, with adjacent thoracic wall structures implied along the rib cage. Superficial veins, intercostal and supraclavicular lymphatic pathways, and segmental cutaneous nerves are integrated around the gland for positional context. Lymphatic drainage is the teaching focus this arrangement supports. Most breast lymph drains laterally to the pectoral (anterior) axillary nodes, then to central and apical nodes, while a medial pathway tracks to parasternal (internal thoracic) nodes, a route that explains contralateral spread and mediastinal involvement in breast carcinoma. It also frames why sentinel lymph node biopsy targets the axilla, and why breast-conserving surgery must respect the relationship between gland, pectoral fascia, and the neurovascular plane that influences sensation and postoperative pain. A practical map.
