Centered at the T8 spinal segment, the cross-section presents the classic H-shaped gray matter surrounded by white matter arranged into posterior (dorsal), lateral, and anterior (ventral) funiculi. Posterior horns extend dorsally toward the posterior median sulcus, while broader anterior horns project ventrally toward the anterior median fissure, with the gray commissure spanning the midline around the central canal. White columns form a thicker peripheral ring than the gray core at this thoracic level, tapering slightly compared with more rostral segments. Fine surface striations suggest bundled longitudinal fibers within the spinal white matter. T8 is a teaching sweet spot because it sits in the mid-thoracic cord where the lateral horn (intermediolateral cell column) is expected, correlating with sympathetic preganglionic neurons from T1 to L2. Cord lesions around this level commonly produce a clear dissociation between ascending dorsal column modalities (gracile fasciculus for lower trunk and leg) and anterolateral system pain and temperature, and the pattern helps localize Brown-Sequard syndrome or anterior spinal artery infarct.
