Description
Centered on a transverse (axial) cross-section of the spinal cord at the T8 segment, the gray matter forms the classic H shape with slender dorsal horns projecting posteriorly and broader ventral horns positioned anteriorly, separated by the gray commissure around the central canal. Surrounding white matter is partitioned into posterior (dorsal), lateral, and anterior funiculi, with the posterior median sulcus and anterior median fissure defining the midline. Within the dorsal columns, the fasciculus gracilis occupies the medial posterior funiculus at this thoracic level, while the dorsolateral sulcus marks the entry zone for dorsal rootlets. Orientation is clear. T8 is a useful teaching level because the dorsal columns here predominantly carry ipsilateral discriminative touch, vibration, and conscious proprioception from the lower trunk and lower limb via the fasciculus gracilis, a relationship that makes segmental localization of myelopathy more than academic. A posterior column lesion above T8 (for example, subacute combined degeneration from vitamin B12 deficiency or dorsal column ischemia) produces loss of vibration and joint position sense below the lesion with sensory ataxia and a positive Romberg sign, while sparing pain and temperature carried in the anterolateral system. The relatively small ventral horns at mid-thoracic levels also help distinguish thoracic cord from cervical and lumbosacral enlargements when correlating with MRI or intraoperative anatomy.
