Description
Seen in transverse section at the T8 segment, the spinal cord displays a narrow, compact butterfly of gray matter surrounded by a relatively thick mantle of white matter. Slender dorsal horns extend posterolaterally toward the dorsal root entry zone, while broader anterior (ventral) horns project anterolaterally toward the ventral rootlets; lateral horns may be suggested at this midthoracic level, reflecting the intermediolateral cell column. A small central canal sits at the gray commissure, with anterior and posterior median sulci marking the midline and helping orient right from left. T8 is a workhorse level for teaching tract topography because the balance of white-to-gray matter makes ascending and descending pathways easy to compare, yet the ventral horns remain small compared with cervical and lumbosacral enlargements. Lesion localization often hinges on this pattern: a syrinx expanding from the central canal first interrupts decussating anterior white commissure fibers (classically producing bilateral segmental pain and temperature loss), while an anterior spinal artery infarct at a thoracic level preferentially injures anterior horn cells and anterolateral funiculi, sparing dorsal column modalities. Small horns, long tracts, big consequences. Use this cross-section in neuroanatomy and medical neuroscience courses when mapping dorsal column, lateral corticospinal, and anterolateral system positions at mid-thoracic levels, or in clinical teaching materials on Brown-Sequard syndrome and central cord syndromes.
