Description
Oriented in transverse section at the T8 spinal cord level, the central gray matter forms the typical H shape around the central canal, with slender posterior (dorsal) horns extending posterolaterally and relatively small anterior (ventral) horns projecting anterolaterally. White matter surrounds this core as posterior, lateral, and anterior funiculi, separated on the midline by the posterior median sulcus dorsally and the anterior median fissure ventrally. At this thoracic level you would also expect a lateral horn (intermediolateral cell column) between dorsal and ventral horns, reflecting preganglionic sympathetic outflow. Segmental dorsal root entry zones lie posterolateral to the dorsal horns, with ventral rootlets exiting anterolaterally. T8 matters because it sits in the thoracic cord where limb motor pools are absent, so the anterior horns are modest compared with cervical and lumbosacral enlargements, while autonomic circuitry remains prominent. Clinically, a lesion around T8, whether from epidural metastasis, disc disease at the corresponding vertebral level, or transverse myelitis, can interrupt long tracts in the lateral funiculus, producing a sensory level on the trunk and upper motor neuron signs below the lesion. Pain and temperature fibers crossing in the anterior white commissure near the central canal also make this level relevant when teaching patterns of central cord syndromes such as syringomyelia. Small horns, big consequences. Use this illustration in neuroanatomy and spinal cord pathway teaching to contrast thoracic segments with cervical and lumbar enlargements, and in neurology or neurosurgery materials that explain tract localization, segmental autonomic outflow, and clinical sensory levels on exam.
