Description
Running distally from the forearm into the carpus, the radial and ulnar arteries course along the volar aspect and contribute to the superficial palmar arch and deep palmar arch across the central palm. From these arches, common palmar digital arteries track distally in the intermetacarpal spaces and divide into proper palmar digital arteries along the medial and lateral margins of each digit toward the phalanges. Metacarpals and phalanges provide the bony scaffold, while the arterial tree overlays them in close apposition to the palmar surfaces and along the axes of the fingers. Small arterioles are suggested as tapering terminal branches within the digits. Palmar arch anatomy matters in the operating room and at the bedside because collateral flow between the radial and ulnar systems varies, and that variation determines how well the hand tolerates arterial cannulation, traumatic laceration, or harvest of the radial artery. Allen test teaching becomes clearer when you can trace the ulnar contribution to the superficial palmar arch and the deeper radial-dominant pathway across the bases of the metacarpals, then follow runoff into the digital vessels. Bleeding here is rarely trivial. Lacerations at the distal palm and along the flexor creases often involve common or proper digital arteries, and repair hinges on knowing where these vessels lie relative to the metacarpals and phalanges.
