Description
Viewed from the distal leg into the ankle and foot, the anterior tibial artery continues across the talocrural joint as the dorsalis pedis artery on the dorsum, while the posterior tibial artery courses posterior to the medial malleolus and divides into the medial and lateral plantar arteries on the sole. Distally, the lateral plantar artery sweeps laterally then turns medially to form the deep plantar (plantar) arch, giving off plantar metatarsal arteries and plantar digital branches that run toward the toes. Proximally and laterally, fibular (peroneal) artery contributions are suggested around the ankle, with perforating connections between plantar and dorsal systems through the intermetatarsal spaces. Bones of the tibia and fibula frame the ankle mortise superiorly, and the tarsals, metatarsals, and phalanges provide clear landmarks for vessel trajectories. Arterial topography at the foot is not an academic nicety, it guides both pulse assessment and limb salvage. Palpation of the dorsalis pedis pulse lateral to the extensor hallucis longus tendon and the posterior tibial pulse posterior to the medial malleolus depends on knowing these surface relationships, and absence or asymmetry often drives workup for peripheral arterial disease. For reconstructive and podiatric surgery, the plantar arch and its metatarsal branches define the vascular territories encountered in plantar incisions, toe amputations, and flap planning, while the ankle level branches matter in diabetic foot ischemia and in choosing targets for distal bypass or endovascular revascularization.
