Description
Medial exposure of the right foot and distal leg brings the tibia into the foreground, with the talus seated superior to the calcaneus and the navicular and cuneiforms stacked along the medial longitudinal arch. The posterior tibial artery courses posterior to the medial malleolus and continues onto the plantar aspect as the medial and lateral plantar arteries, while the dorsalis pedis artery on the dorsum contributes via the deep plantar branch to the plantar arterial arch beneath the metatarsal bases. Small arterioles track distally toward the hallux and medial forefoot, running in close apposition to the tarsus and first metatarsal. Veins accompany the arteries in the usual paired pattern. Clear bony landmarks. Clinically, the relationship of the posterior tibial artery to the flexor retinaculum and tarsal tunnel matters when you are correlating a palpable posterior tibial pulse with neurovascular compression patterns in tarsal tunnel syndrome. Reconstructive and diabetic foot surgery also depend on this map: the plantar arterial arch and its metatarsal branches determine flap viability and guide incision planning along the medial arch to avoid compromising inflow to the hallux and forefoot.
