Description

Medial aspect of the male foot and distal leg is rendered with the tibia descending to the medial malleolus and the tarsal and metatarsal framework visible beneath the vascular map. The posterior tibial artery courses posterior to the medial malleolus within the tarsal tunnel, then divides into the medial and lateral plantar arteries deep to abductor hallucis as it enters the sole. Distally, plantar branches track along the medial longitudinal arch toward the hallux, with accompanying venae comitantes and a superficial venous channel along the medial border leading toward the great saphenous vein. Bony landmarks anchor the relationships. Medial vascular anatomy at the ankle matters because so many diagnoses and procedures cluster around this corridor. The posterior tibial pulse is palpated between the medial malleolus and the calcaneal tuberosity, and loss of this pulse is a practical bedside marker in peripheral arterial disease and acute limb ischemia. For reconstructive and diabetic foot care, the branching pattern of the medial and lateral plantar arteries guides flap planning and incision placement, and it also explains why tarsal tunnel decompression and medial hindfoot approaches must respect the neurovascular bundle.
