Description
Bilateral male kidneys are presented flanking the abdominal aorta on the left and the inferior vena cava on the right, with the renal arteries arising laterally from the aorta and the renal veins coursing medially to drain into the cava. At each renal hilum, the artery and vein sit anterior to the renal pelvis, while the ureter descends inferiorly from the pelvis as a beige tubular continuation. Perihilar and perirenal fat is rendered as a distinct envelope around the central branches, framing the hilar structures and tracking toward the renal sinus. Green neurovascular elements likely represent renal plexus nerves and or lymphatic channels accompanying the renal vessels. This arrangement matters because the renal pedicle is where anatomy becomes operative, and small errors in spatial understanding translate into hemorrhage, ureteric injury, or postoperative urinoma. Laparoscopic nephrectomy, donor nephrectomy, and renal transplantation all hinge on recognizing the short right renal vein entering the inferior vena cava, the longer left renal vein crossing to the cava, and the branching pattern of the renal artery near the hilum, often obscured by renal fat. Perirenal fat also anchors the kidney within Gerota’s fascia and becomes clinically relevant in perinephric abscess, retroperitoneal hemorrhage, and the staging of renal cell carcinoma when tumor extends into perinephric tissues.
