Description

Paired kidneys are shown in situ with the abdominal aorta and inferior vena cava running vertically in the midline posterior to the peritoneal cavity, framed by a segment of lumbar vertebral column. From the aorta, the right and left renal arteries course laterally to the renal hilum, where they divide into segmental, interlobar, arcuate, and interlobular branches that radiate through the renal sinus into the cortex and medulla. Venous tributaries converge in the reverse order toward the renal veins, which drain medially into the inferior vena cava, while the renal pelvis lies posterior to the main hilar vessels and narrows inferiorly into the ureter. Emphasis on the intrarenal branching pattern matters because it maps directly onto the kidney’s segmental vascular territories, the anatomic basis for partial nephrectomy and segmental arterial clamping to limit ischemia. The relationship of the renal pelvis and proximal ureter to the hilar vessels also frames common pain and obstruction scenarios, from ureteropelvic junction obstruction to stone impaction, and clarifies why crossing vessels can contribute to intermittent hydronephrosis. Keep an eye on laterality. The shorter right renal artery often passes posterior to the inferior vena cava, a consistent surgical orientation point in right-sided hilar dissection.
