Description
Semi-transparent skin reveals the peripheral nerves coursing from the distal leg into the left foot, with the tibial nerve passing posterior to the medial malleolus and dividing into the medial and lateral plantar nerves deep to the flexor retinaculum. Distally, plantar metatarsal nerves give rise to common and proper plantar digital nerves that run anteriorly toward the toes, while calcaneal branches sweep posteriorly to the heel pad. Along the lateral border, the sural nerve continues as the lateral dorsal cutaneous nerve, and the superficial fibular (peroneal) nerve supplies the dorsum with dorsal digital rami; medially, the saphenous nerve tracks along the medial malleolus toward the medial forefoot. Color-coded yellow rami clarify proximal to distal continuity. Pedal neuroanatomy becomes clinically concrete at the tarsal tunnel, where tibial nerve compression produces burning plantar pain and paresthesia in the medial and lateral plantar distributions and can be provoked by ankle eversion and dorsiflexion. That branching pattern also guides local anesthetic placement for posterior tibial nerve blocks and helps explain why interdigital symptoms in Morton neuroma typically localize to the third common plantar digital nerve between the third and fourth metatarsal heads. Small branches matter. Misidentifying them drives incomplete anesthesia and wrong-site decompression.
