Description

Arising from the distal leg, the tibial nerve passes posterior to the medial malleolus within the tarsal tunnel and divides into the medial and lateral plantar nerves that fan anteriorly across the plantar tarsus and metatarsals toward the toes. Laterally, the common fibular (peroneal) nerve continues as the superficial fibular nerve over the dorsum of the foot, while the deep fibular nerve courses more centrally with the dorsalis pedis artery region toward the first web space. Cutaneous branches consistent with the saphenous nerve medially and sural nerve posterolaterally frame the ankle and hindfoot, with digital nerves tracking distally along the medial and lateral margins of each toe. Dense branching dominates the pedal forefoot. For teaching and clinical work, this is the money zone: the posterior tibial nerve and its bifurcation under the flexor retinaculum, where entrapment produces tarsal tunnel syndrome with plantar paresthesia and burning pain radiating into the medial and lateral plantar distributions. The plantar course also maps directly onto regional anesthesia, including posterior tibial nerve blocks at the level of the medial malleolus and digital blocks along the sides of the toes for nail surgery and forefoot procedures. Dorsally, the deep fibular nerve territory in the first interdigital cleft remains a useful sensory checkpoint in L5 radiculopathy and anterior compartment syndrome workups.
