Description
Arising from the superficial venous plexus posterior to the auricle, the posterior auricular vein courses inferiorly over the mastoid region, running superficial to the insertion of sternocleidomastoid on the mastoid process and posterior to the external acoustic meatus. Its tributaries track along the posterior scalp margin and retroauricular soft tissues, where small venules converge into a single named vessel. Inferiorly, it typically joins the posterior division of the retromandibular vein to form, or contribute to, the external jugular vein as it descends across the lateral neck. Orientation is clear. For otologic and mastoid surgery, this vessel matters because it lies in the operative field of postauricular approaches used for mastoidectomy and cochlear implantation, where brisk venous bleeding can obscure the periosteum and mastoid cortex during flap elevation. Its relationship to the mastoid emissary veins and the sigmoid sinus region also helps explain potential venous pathways for infection spread from posterior scalp or external ear toward intracranial dural sinuses.
