Description
Arising from the scalp over the posterior occiput, the occipital vein descends in the posterior midline region, superficial at first and then tracking deep to the posterior cervical musculature as it approaches the upper neck. Relative to the external occipital protuberance and superior nuchal line, the vessel runs inferiorly toward the suboccipital region, lying posterior to the cervical vertebral column while remaining lateral to the ligamentum nuchae. Small venules and branching rami accompany it in the occipital scalp, and adjacent arterial counterparts and segmental vessels appear in parallel pathways against the bony landmarks of the occipital bone and cervical vertebrae. Posterior venous anatomy in the head and neck matters when you need to predict bleeding planes and safe corridors during occipital scalp surgery, posterior cervical exposure, or flap elevation. The occipital vein communicates variably with the suboccipital venous plexus and deeper cervical venous channels, a pattern that explains brisk venous oozing in the nuchal musculature even when the superficial scalp vein looks controlled. It also helps orient learners to relationships between vessels and the greater occipital nerve in the same territory. A frequent site of confusion.
