Description
Arising from the common tendinous ring at the orbital apex, the superior rectus runs anteriorly in a straight course along the superior surface of the globe to its scleral insertion just posterior to the corneoscleral limbus. The muscle belly sits superior to the optic nerve and superior ophthalmic vein region, with the levator palpebrae superioris expected immediately superior to it in the roof of the orbit, separated by a thin fascial plane. Adjacent extraocular muscles frame the globe, with the medial and lateral rectus flanking it on the nasal and temporal sides and the inferior rectus opposing it inferiorly, while the oblique muscles course more obliquely around the posterior globe. A cross-section style cut emphasizes the relationship of the rectus cone to the sclera and posterior pole. Superior rectus anatomy matters because small errors in localization translate into real motility problems. Injury to the superior division of the oculomotor nerve weakens elevation and can mimic or coexist with superior rectus restriction, while thyroid eye disease commonly enlarges rectus muscles and produces restrictive diplopia that is often characterized clinically with forced duction testing. Surgical planning in strabismus hinges on its insertion and arc of contact, and the proximity to the levator complex explains why postoperative ptosis can follow superior rectus recession or transposition.
