Description
Occupying the superior aspect of the orbit, the superior rectus muscle runs from the common tendinous ring (annulus of Zinn) at the orbital apex anteriorly to its insertion on the superior sclera, just posterior to the corneoscleral limbus. The muscle belly lies immediately inferior to the orbital roof and superior to the globe, with its tendon crossing the superior surface of the eyeball in line with the visual axis. Posteriorly, the optic nerve exits the globe and courses back toward the optic canal, medial to the lateral rectus and deep to the extraocular muscle cone. Bone from the orbital margins frames the conical orbital space. Clear orientation. Primary action is elevation of the adducted eye, but this view also reinforces the clinically relevant secondary actions: intorsion and adduction because the superior rectus lies at an angle to the anteroposterior axis of the globe. That relationship matters when teaching or interpreting ocular motility testing, where limitation of upgaze may reflect superior rectus paresis from an oculomotor nerve (CN III) palsy, restrictive thyroid eye disease, or postoperative scarring. The proximity of the muscle cone to the optic nerve also anchors discussions of orbital apex syndromes and compressive optic neuropathy.
