Description
Arcing across the lateral midface, the zygomatic branch of the facial nerve (CN VII) courses anterior to the auricle and parotid region toward the zygomatic (malar) bone, then spreads superiorly and medially over the zygomatic arch toward the lateral canthus. Superficial to the masseter and lateral to the maxilla, it lies in the same territory as the zygomatic and buccal branches, with nearby sensory pathways of the trigeminal nerve (CN V), including the maxillary division (V2) across the infraorbital region. Bony landmarks of the orbit, maxilla, and mandible frame the nerve’s route. Teeth and the nasal and oral cavities are also visible. Clinically, this branch matters because it carries motor fibers to the orbicularis oculi and adjacent zygomaticus musculature, so iatrogenic injury can present as incomplete eyelid closure, reduced blink, and tearing problems rather than numbness. A common teaching point is the close but separate relationship between facial nerve motor branches and trigeminal sensory branches in the cheek, an issue that surfaces during parotidectomy, midface laceration repair, or facelift (rhytidectomy) dissection near the zygomatic arch. Surface anatomy helps. The zygomatic branch is typically encountered in the superficial musculoaponeurotic system plane as it crosses the malar eminence toward the periorbital region. Plastic surgery and ENT training programs can use this artwork to explain safe dissection planes over the malar bone and to map zones of risk for facial nerve injury during parotid and midface procedures.
