Description
Sweeping across the posterolateral scalp, the occipitofrontalis (epicranius) is presented immediately deep to the skin, with emphasis on the occipital belly over the superior nuchal region and adjacent occipital bone. Anteriorly it blends into the epicranial aponeurosis (galea aponeurotica), which spans toward the frontal belly, while posteriorly the muscle fibers converge toward their attachment along the lateral two thirds of the superior nuchal line and the mastoid region. Inferior to the occipital belly, the nuchal musculature is visible in context, with the trapezius forming a broad superficial sheet over the upper back and the sternocleidomastoid defining the lateral contour of the neck. Scalp anatomy is easiest to teach when the occipitofrontalis is kept in place relative to the galea and the overlying skin, because the scalp’s mobility, wrinkle patterns, and wound gaping depend on this continuous musculoaponeurotic layer. Clinically, this relationship explains why lacerations that violate the galea tend to retract and bleed briskly, and why subgaleal hematoma can dissect widely across the calvaria under the aponeurosis. The posterior belly also provides a practical entry point for discussing facial nerve supply via the posterior auricular branch and for differentiating it from the neighboring posterior neck muscles during flap elevation.
