Description
Posterior head and upper cervical region are presented with the occipital belly of the occipitofrontalis (epicranius) highlighted over the occipital squama, spanning laterally toward the mastoid region. Superiorly, its fibers blend into the galea aponeurotica (epicranial aponeurosis), which continues anteriorly toward the frontal belly across the cranial vault. Inferior to the occipitalis, the upper trapezius forms the superficial contour of the posterior neck, while the sternocleidomastoid borders the lateral cervical region as it ascends toward the mastoid process. Fiber direction is clear. Landmarks matter. Clinically, this posterior perspective matters when you are teaching scalp mobility and the layered anatomy that governs bleeding and spread of infection: skin, dense connective tissue, aponeurosis, loose areolar tissue, and pericranium. The occipitalis anchors the scalp to the superior nuchal line, so lacerations that violate the epicranial aponeurosis can gape widely, and avulsions may track in the loose areolar plane, producing extensive posterior scalp hematomas. It also supports discussions of facial nerve anatomy, since occipitalis receives motor supply from the posterior auricular branch of the facial nerve (CN VII), a point that helps explain asymmetry of scalp movement after parotid or mastoid surgery even when the cervical plexus remains intact. Use this illustration in gross anatomy and head and neck modules to reinforce posterior scalp layers, muscle attachments to the occipital bone, and surface relationships to trapezius and sternocleidomastoid.
