Description
Beneath the posterior scalp of an adult male, the occipital belly of the occipitofrontalis (epicranius) fans out from the lateral two thirds of the superior nuchal line and adjacent mastoid region, then courses superiorly to blend with the galea aponeurotica (epicranial aponeurosis). The aponeurosis spans the calvaria in the midline, lying superficial to the pericranium and deep to the skin, while the frontal belly would continue anteriorly beyond the field of this posterior view. Inferior to the occiput, the trapezius forms a broad superficial sheet over the posterior neck and upper thorax, with splenius capitis and semispinalis capitis inferred deep and medial, and levator scapulae tracking down toward the superior angle of the scapula. Fiber direction is clear. For teaching scalp mechanics, this posterior perspective clarifies how contraction of the occipital belly tensions the galea and contributes to posterior scalp retraction while the frontal belly elevates the eyebrows, a paired action often tested in facial nerve (CN VII) examination. The loose areolar tissue deep to the galea explains why scalp lacerations can gape widely and bleed profusely, and why avulsion injuries may separate the scalp as a single flap along this plane. Surgeons planning coronal or occipital incisions also need these relationships when elevating scalp flaps, protecting the vascular supply from the occipital artery and avoiding unnecessary traction on the posterior auricular branch of the facial nerve.
