Posterior elements of the male cervical spine are arranged in stacked sequence from the atlas (C1) through the lower cervical vertebrae, with the midline spinous processes aligned inferiorly toward C7. At C1, the posterior arch forms a short bony bridge between the lateral masses, framing the vertebral foramen and sitting superior to the axis (C2). Moving caudally, the bifid tips of the spinous processes become apparent in the typical lower segments (classically C3 to C6), while the transverse processes project laterally on each side as muscle and ligament attachment sites. Posterior cervical anatomy matters because it is the corridor you navigate for many interventions and for reading trauma patterns on radiographs and CT. The atlas is a frequent focus: fractures through the anterior and posterior arches (Jefferson burst fracture) follow axial loading, and the relationship of the lateral masses to the occipital condyles at the superior articular facets underpins atlanto-occipital stability. Palpation and surgical exposure also track these landmarks, from the C2 spinous process down to the bifid processes that can guide level identification during posterior approaches.
