Description
Posteriorly, the cardiac silhouette is dominated by the left atrium at the base, with the four pulmonary veins entering its posterior wall on either side, shown in blue to emphasize venous return. Inferior to the left atrium, the diaphragmatic surface of the left ventricle occupies most of the posterior aspect, while the right atrium lies more to the anatomical right and receives the superior and inferior venae cavae. The coronary sinus courses in the posterior atrioventricular (coronary) sulcus and drains into the right atrium, with the posterior interventricular sulcus marking the boundary between ventricles as it descends toward the apex. Clear landmarks. This posterior perspective is the one you want when teaching why the left atrium forms most of the heart’s base and why pulmonary venous anatomy matters in the electrophysiology lab. Variants in pulmonary vein ostia and a dilated left atrium are central to atrial fibrillation mapping and pulmonary vein isolation, and the relationship of the coronary sinus to the mitral annulus underpins both mitral valve surgery and coronary sinus cannulation for retrograde cardioplegia.
