Description
Cutaneous layers are rendered in cross-section with an emphasis on an actinic keratosis focus at the surface. An irregularly thickened stratum corneum caps the lesion, with parakeratosis extending downward over a disordered viable epidermis where atypical keratinocytes cluster along the basal and suprabasal layers. Beneath, the papillary and reticular dermis contain adnexal structures, including hair follicles with associated sebaceous glands and deeper eccrine sweat coils, while small arteries and veins course superior to the hypodermal adipose lobules. UV injury dominates. Actinic (solar) keratosis sits on the pathway to cutaneous squamous cell carcinoma, so morphology matters: the combination of hyperkeratosis, parakeratosis, and basal keratinocyte atypia is the pattern clinicians correlate with a rough, adherent scale on sun-exposed skin and pathologists use to grade dysplasia and decide when a biopsy is warranted. The dermal context supports discussion of chronic photodamage, including solar elastosis and superficial vascular ectasia that often accompany these lesions and influence treatment selection across a field of damaged epidermis. Cryotherapy margins, curettage depth, and the expected response to topical 5-fluorouracil, imiquimod, or photodynamic therapy all hinge on understanding where the atypia resides relative to the dermoepidermal junction and follicular epithelium.
