Description
Rendered as a cross-sectional block of human skin, the actinic keratosis focus sits on the surface as a hyperkeratotic, scaly plaque overlying an erythematous epidermis. The thickened stratum corneum caps an epidermis with disordered keratinocyte maturation extending from the stratum basale toward the stratum spinosum, while the underlying dermis contains hair follicles with emerging shafts, associated sebaceous glands, and coiled eccrine sweat glands deeper in the reticular dermis. Vessels course within the dermis beneath the lesion, and a lobulated hypodermis of adipose tissue anchors the integument inferiorly. Clinically, this is the classic architecture of a solar (actinic) keratosis on chronically sun-exposed skin, where ultraviolet damage drives epidermal dysplasia that can progress to cutaneous squamous cell carcinoma, a risk that rises with tenderness, rapid thickening, ulceration, or a persistent indurated base. The cross-section also helps teach the histopathologic correlates often discussed at biopsy: compact hyperkeratosis with focal parakeratosis, atypical basal keratinocytes, and solar elastosis in the superficial dermis adjacent to adnexal structures. Small details matter, because follicular extension of atypia can explain incomplete clearance after superficial treatments.
