Description
Sun-damaged epidermis is rendered in cross-section with an actinic keratosis focus marked by surface hyperkeratosis overlying a thickened stratum corneum and disordered keratinocyte maturation within the lower epidermal layers. Basal and suprabasal keratinocytes appear atypical relative to adjacent epidermis, while the undulating dermoepidermal junction forms prominent dermal papillae beneath the lesion. Hair follicles descend from the epidermis into the dermis with attached sebaceous glands, and eccrine sweat coils sit deeper, surrounded by small-caliber vessels and cutaneous nerves; subcutaneous adipose occupies the most inferior plane. Actinic keratosis sits on the continuum toward squamous cell carcinoma in situ, and this sectional presentation makes that teaching point concrete by localizing atypia to the epidermis while keeping the basement membrane intact. Solar elastosis and superficial vascular ectasia in the papillary dermis help anchor the diagnosis to chronic ultraviolet exposure rather than an isolated keratotic papule.
