Anterior Perspective of the Cricotracheal Ligament

Centered in an anterior neck view, the thyroid cartilage forms the superior, shield-like contour, with the cricoid cartilage immediately inferior and the first tracheal ring continuing caudally into the trachea. Between the inferior border of the cricoid arch and the superior margin of the uppermost tracheal cartilage sits the cricotracheal ligament, a short band of fibroelastic tissue spanning the midline and blending laterally with the perichondrium. Superior to the laryngeal framework, the epiglottis projects behind the hyoid region, while the infrahyoid strap muscles lie anterolaterally and frame the larynx along the midline. Cartilaginous elements and adjacent soft tissues are differentiated by color for rapid orientation. For airway surgery and emergency access, the cricotracheal junction is a practical landmark because it marks the transition from laryngeal to tracheal cartilaginous support and sits just inferior to the cricoid, the only complete ring in the adult airway. Misidentification at this level can shift an incision too cephalad toward the cricothyroid membrane or too caudad between tracheal rings, each with different risks for subglottic injury, bleeding from anterior jugular venous channels, and postoperative stenosis.
