Description
From an anterior (palmar) perspective of the male wrist and hand skeleton, the transverse carpal ligament (flexor retinaculum) spans the volar aspect of the carpus as a dense band bridging the carpal arch. Medially, it courses from the pisiform and hook of hamate, and laterally it anchors to the tubercle of the scaphoid and the ridge of the trapezium, lying superficial to the proximal carpal row and proximal to the metacarpals. Distal phalanges, middle and proximal phalanges, and the five metacarpals extend distally from the carpus with the metacarpophalangeal and interphalangeal joints aligned in their usual longitudinal axes. Carpal relationships remain clear, with scaphoid and lunate lateral to triquetrum and pisiform, and trapezium, trapezoid, capitate, and hamate forming the distal row. Palmar anatomy makes sense when you can see the ligament that turns the concave carpal bones into a tunnel. The transverse carpal ligament forms the roof of the carpal tunnel, converting the carpal sulcus into a fibro-osseous canal for the median nerve and flexor tendons, and its thickness and attachments explain why carpal tunnel syndrome presents with nocturnal paresthesias in the radial three and a half digits and why release is performed along the ulnar side of the median nerve. It also clarifies adjacent spaces such as Guyon’s canal, where the ulnar nerve runs superficial and medial to the retinaculum near the pisiform and hamate. A tight, unforgiving roof.
