Description
Arising from the L1 spinal nerve, the genitofemoral nerve emerges on the anterior surface of the psoas major and descends inferiorly toward the inguinal region in an anterior perspective of the male trunk. A short distance proximal to the inguinal ligament it divides into a genital branch and a femoral branch, the split typically occurring medial to the iliacus and lateral to the great vessels. The genital branch tracks anteroinferiorly toward the deep inguinal ring and spermatic cord, while the femoral branch passes beneath the inguinal ligament to supply the skin over the femoral triangle. Small nerve. Big consequences. That branching pattern matters when you are sorting groin and anterior thigh sensory symptoms after retroperitoneal or inguinal surgery. The nerve is exposed to stretch or transection during psoas retraction in anterior approaches to the lumbar spine, and it can be injured during hernia repair or pelvic lymphadenectomy where it runs close to the deep inguinal ring. Differentiating genitofemoral neuralgia from ilioinguinal or iliohypogastric nerve pain hinges on understanding where the genital branch follows the cord and where the femoral branch stays lateral to the pubic tubercle under the inguinal ligament.
