Iliohypogastric Nerve, Anterior View

Description

Arising from the L1 ventral ramus, the iliohypogastric nerve tracks laterally across the anterior surface of quadratus lumborum and then runs superior to the iliac crest on the internal aspect of the ilium before coursing anteriorly between transversus abdominis and internal oblique. Its trajectory sits superior to the ilioinguinal nerve, which follows a more inferior and medial line toward the inguinal canal. Bony landmarks such as the iliac crest, anterior superior iliac spine, lower ribs, lumbar vertebrae, and sacrum anchor the nerve’s position in the lower abdominal wall. Orientation matters. Clinically, this superior course makes the iliohypogastric nerve a frequent casualty in lower abdominal incisions, including Pfannenstiel and gridiron approaches, and in trocar placement during laparoscopic appendectomy or hernia repair when ports stray too lateral. Injury or entrapment can produce postoperative neuropathic pain and hypoesthesia over the suprapubic skin and superolateral gluteal region, and it is often misattributed to ilioinguinal neuralgia unless you keep their relative positions straight. A clean anterior depiction helps clarify where the nerve pierces the internal oblique and where its anterior cutaneous branch emerges near the rectus sheath.
