Description
Running inferiorly from the upper poles of the thyroid gland, the superior thyroid veins course anterolateral to the larynx and upper trachea before converging toward the carotid sheath. Each vein lies in close company with the superior thyroid artery at the superior pole, while the internal jugular vein runs more lateral and posterior, receiving this drainage near its proximal cervical segment. Medially, the paired thyroid lobes straddle the trachea and join across the midline by the isthmus. Short venous tributaries from the superior pole and adjacent infrahyoid region are implied in the confluence. Surgical anatomy in this zone is unforgiving. During thyroidectomy or emergent cricothyrotomy performed too low, brisk bleeding can come from the superior thyroid venous pedicle at the upper pole or from communicating veins near the midline, and traction on the superior pole brings the external branch of the superior laryngeal nerve into the same working corridor as the superior thyroid vessels. Clear depiction of the superior thyroid veins in anterior relation to the strap muscles and laryngotracheal framework helps plan safe ligation close to the gland, where the nerve is least exposed.
