Posterior uterine anatomy is presented as an external view of the uterus (womb), emphasizing the facies intestinalis (posterior surface) of the fundus and body. The serosal peritoneal covering forms a smooth continuous layer over the convex posterior contour, extending inferiorly toward the isthmus and cervix. Superiorly and posteriorly, this peritoneum reflects onto adjacent pelvic viscera, creating the rectouterine pouch (pouch of Douglas) between uterus and rectum. Spatially, the broad posterior surface faces posteroinferiorly within the pelvis, with the fundus positioned superior to the cervix and the uterine body anterior to the rectum. This posterior aspect matters in operative gynecology because the peritoneal reflections and the rectouterine pouch define where fluid, blood, or pus collects in the dependent female pelvis, and where access may be obtained via culdocentesis through the posterior vaginal fornix. It is also the surface encountered during posterior dissection in hysterectomy, endometriosis surgery, and adhesiolysis when bowel loops adhere to the uterine serosa. A frequent teaching point. Posterior uterine serosa provides a clean landmark for distinguishing intraperitoneal from extraperitoneal planes as the surgeon works toward the cervix and uterosacral ligament region. Use this rendering in pelvic anatomy lectures to orient students to uterine surfaces (facies vesicalis versus facies intestinalis), peritoneal folds, and the relationship of the uterus to the rectum and pelvic peritoneal spaces.
