Description
Posterior (intestinal) surface of the uterus is rendered in external view, emphasizing the facies intestinalis uteri and the posterior myometrial contour relative to the smoother, more anteriorly convex vesical surface. Superiorly, the fundus uteri rounds into the uterine body, while inferiorly the contour narrows toward the isthmus and cervix region. Laterally, the uterine margins imply the broad ligament attachment zones and the uterine horns where the uterine tubes would originate, even if adnexal structures are not the focus. Posterior wall thickness matters when you are teaching uterine orientation and palpation landmarks, and it also frames common posterior compartment pathology. Deep infiltrating endometriosis often involves the posterior uterine serosa, the rectouterine pouch (pouch of Douglas), and the uterosacral ligaments, so a clean posterior surface view helps correlate pain patterns with the anatomic relationships to rectum and sigmoid colon. During total laparoscopic hysterectomy or uterine-sparing surgery, appreciating the posterior uterine curvature and the transition toward the cervix guides dissection planes near the rectovaginal septum and reduces the risk of rectal injury.
