Seen from a posterior orientation, the uterus is presented with emphasis on the facies intestinalis (posterior surface) of the corpus uteri, covered by a smooth peritoneal reflection that continues inferiorly toward the isthmus and cervix. Superiorly, the fundus forms the rounded dome, while the posterior wall slopes inferiorly to the cervix, which lies more inferior and slightly posterior relative to the bladder-facing anterior surface. Laterally, the uterine margins broaden toward the cornua, where the uterine tubes would originate and where the broad ligament attaches along the uterine sides. The overall geometry reads as a midline pelvic organ with a posterior peritoneal coat facing the rectum and sigmoid colon. This posterior view matters because it is the surface you meet when approaching the uterus through the pouch of Douglas (rectouterine pouch) at laparoscopy, where adhesions, endometriotic implants, or inflammatory scarring often tether the posterior uterine serosa to the rectosigmoid.
